We are committed to complying with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the Health Information Technology for Economic and Clinical Health Act (HITECH) and their implementing regulations. This Privacy Notice describes how we may use and disclose protected health information (PHI) about you and your patients. It also describes your rights and our obligations with respect to the use and disclosure of PHI.

What is Protected Health Information?

Protected health information (PHI) is individually identifiable health information that is transmitted or maintained in any form or medium, whether electronic, paper, or oral. PHI includes demographic information that identifies an individual, such as name, address, and birth date, and other information that relates to the individual’s past, present, or future physical or mental health or condition and related health care services. PHI also includes information that identifies the individual or for which there is a reasonable basis to believe it can be used to identify the individual.

I'm a patient and want my testimonial removed.

Patients have the right to withdraw their consent form and ask to get the testimonial removed under the laws of the HIPAA.

If you're a patient and you want your testimonial removed, please send us an email to support@socialjuice.io telling us you want to get the testimonial removed. Please use the same email address you used to send the testimonial, so we can easily check you are the one who left it. Also mention the link of the testimonial collection page.
